1). Insurance Information:

If you have one of the following insurance plans, please call our office at (770) 948-0036 to schedule your annual eye exam:

• Medicaid
• Amerigroup
• Peach State
• CareSource
• VA Insurance

(Online scheduling is not available with these plans.)

2). Office Policies

Please Note: To provide the most thorough evaluation of your eye health, all eye examinations include an Optomap® retinal image. The fee for this advanced imaging service is $44. Most vision insurance plans do not cover retinal imaging; therefore, patients are responsible for the $44 fee.

A 3% convenience fee will be added to payments made by debit or credit card. This fee does not apply to payments made by cash, check, Health Savings Account (HSA) cards, or Flexible Spending Account (FSA) cards.

3). Medical issues:

If you are experiencing any medical issue related to your eyes, please call us directly at (770) 948-0036 to schedule your appointment.

This includes, but is not limited to:

• Eye infection
• Flashes of Light
• Sudden onset of floaters
• Vision loss
• Pain
• Redness or swelling
• Any other urgent eye concern

Calling us directly at (770) 948-0036 allows us to triage your condition and get you scheduled as quickly as possible.
